Job summary

Qualifications

• RN with active license in practicing state.
• 2+ years in health care, care management or LTSS preferred.
• Valid driver’s license and reliable transportation for travel.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans in collaboration with members and providers.
• Monitors care plan effectiveness and documents progress toward goals.
• Promotes integrated care across behavioral health and LTSS.
• Assesses medical necessity and coordinates authorization for waiver services.
• Coordinates ICT meetings for service approvals and collaboration.
• Travel 25-40% locally as required.
