Qualifications

• At least 2 years of health care experience, including 1 year in care management, managed care, or LTSS.
• RN license must be active and unrestricted.
• Bachelor’s degree may be required in some states/contractual contexts.
• Valid driver’s license and reliable transportation for travel.
• Ability to operate proactively with attention to detail and self-motivation.
• Strong communication skills (verbal and written).
• Experience with individuals with disabilities or chronic conditions is preferred.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including in-person home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians, and other health professionals.
• Monitor care plans and adjust interventions to improve outcomes and cost-effectiveness.
• Promote integration of services across behavioral health and LTSS, leveraging community resources.
• Assess medical necessity and authorize appropriate waiver services.
• Participate in ICT meetings for service decisions and collaboration.
• Conduct medication reconciliation as needed and handle barriers to care; may travel 25–40% locally.
