Qualifications

• Licensed nurse in the state
• Medication administration experience
• Familiarity with PCC EMR (Point Click Care)
• Memory care techniques and protocols
• Knowledge of MDS assessments in long-term care
• Experience with SNF, sub-acute, and dementia units
• Background in case management practices
• Ability to work in a medical office environment
• Strong communication with patients, families, and teams
• Solid organization to manage multiple tasks

Responsibilities

• Assess resident health problems and needs; develop care plans
• Administer medications and treatments per physicians
• Monitor resident progress and respond to changes
• Collaborate with multidisciplinary teams to deliver comprehensive care
• Educate residents and families on health conditions and discharge plans
• Maintain accurate resident records in compliance with regulations including HIPAA
• Participate in case management to ensure continuity of care
• Support in PT/OT/SUD, Pain Management, and related areas
