AuthentiCare Health Centers in Massachusetts is seeking compassionate RN or LPN for our skilled nursing facilities in Lawrence. Join our caring family and help deliver high-quality long-term, sub-acute, and memory care with resident-centered approaches.

We offer a $5,000 sign-on bonus, weekly pay, tuition reimbursement, and an employee assistance program. You will assess, plan, administer medications, and collaborate with a multidisciplinary team to support residents and families.

Qualifications

• Current RN or LPN license in the state.
• Experience administering medications and treatments.
• Proficiency with PCC EMR system.
• Experience in memory care techniques and dementia units.
• Familiar with MDS assessments in long-term care.
• Strong case management and coordination skills.
• Ability to collaborate with multidisciplinary teams.
• Excellent communication with residents, families and healthcare teams.

Responsibilities

• Assess resident health problems and develop nursing care plans.
• Administer medications and treatments as prescribed.
• Monitor resident progress and respond to changes.
• Collaborate with multidisciplinary teams for comprehensive care.
• Educate residents and families about health conditions and discharge.
• Maintain resident records in compliance with HIPAA and regulations.
• Participate in case management to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, Pain management.
