Royal Falmouth Nursing and Rehabilitation Center in Teaticket, MA, is seeking an RN or LPN to deliver resident-centered long-term, sub-acute, and memory care. You will assess, plan, administer medications, and collaborate with a multidisciplinary team in a supportive environment.

Qualifications

• Must be a Registered Nurse or Licensed Practical Nurse in Massachusetts.
• Experience with medication administration.
• Familiar with PCC EMR system.
• Knowledge of memory care techniques and protocols.
• Understanding of MDS assessments for long-term care facilities.
• Experience in Skilled Nursing, Sub-Acute, and Dementia units.
• Strong case management background.
• Ability to work in a medical office environment.
• Excellent communication with patients, families, and healthcare teams.
• Strong organizational skills.

Responsabilités

• Assess resident health problems and needs, develop and implement nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families regarding health conditions, treatment plans, and discharge.
• Maintain accurate resident records and documentation in compliance with state and federal regulations, including HIPAA.
• Participate in case management activities to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.
