Qualifications

• Must be a registered or licensed nurse in Massachusetts.
• Experience administering medications and treatments.
• Proficient with PCC EMR and memory care protocols.
• Knowledge of MDS assessments for long-term care.
• Experience in Skilled Nursing, Sub-Acute and Dementia units.
• Strong case management and coordination skills.
• Excellent communication with patients, families, and healthcare teams.

Responsibilities

• Assess resident health problems and develop care plans.
• Administer medications and treatments as prescribed.
• Monitor resident progress and respond to changes.
• Collaborate with multidisciplinary teams for comprehensive care.
• Educate residents and families about conditions and plans.
• Maintain accurate documentation in compliance with HIPAA.
• Support in case management to ensure continuity of care.
• Assist in specialized areas such as PT/OT/SUD and pain management.
