You will deliver high-quality resident-centered care across long-term, sub-acute, and memory care settings, contributing to a supportive and collaborative environment.

Qualifications

• Licensed RN or LPN in Massachusetts with active license.
• Experience in long-term, sub-acute, and memory care settings.
• Ability to provide resident-centered care and education.

Responsibilities

• Assess resident health problems and develop care plans.
• Administer medications and treatments as prescribed.
• Monitor resident progress and respond to changes.
• Collaborate with multidisciplinary teams for comprehensive care.
• Educate residents and families on health conditions and discharge plans.
• Maintain accurate resident records in compliance with HIPAA.
• Support case management for continuity of care.
• Assist in specialized areas such as PT, OT, SUD, and pain management.
