Qualifications

• Licensed as a registered nurse in the applicable state
• Experience in long-term care, sub-acute, or memory care settings
• Proficient in EMR systems and HIPAA-compliant documentation
• Knowledge of MDS assessments for long-term care facilities

Responsibilities

• Assess resident health problems and needs, develop and implement nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families about health conditions, treatment plans, and discharge.
• Maintain accurate resident records in compliance with state and federal regulations.
• Participate in case management to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, and Pain Management.
