Qualifications

• RN or LPN licensure in Massachusetts.
• Experience in long-term/sub-acute/memory care preferred.
• Proficiency with PCC EMR and Medicaid/MDS processes.

Responsibilities

• Assess resident health problems and develop nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families on health conditions, plans, and discharge.
• Maintain accurate resident records compliant with HIPAA and regulations.
• Participate in case management to ensure continuity of care.
• Support in areas such as PT, OT, SUD, Pain Management.
