Job summary

Commonwealth Care Alliance in Massachusetts is seeking an Integrated Care Clinical Manager (RN) to monitor, follow up, and manage care for dually-eligible enrollees with complex medical, behavioral, and social needs.

This mobile role coordinates with primary care physicians, specialists, and community resources to integrate health services and improve outcomes.

You will lead the ICT, advocate for enrollees, and support preventive care and chronic disease management.

Qualifications

• RN licensed in MA with ability to manage complex cases.
• Experience with dual-eligible populations preferred.
• Knowledge of Medicare/Medicaid programs helpful.

Responsibilities

• Engage with enrollees in homes and community settings to establish care relationships.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate with providers and community resources to avoid duplication.
• Monitor enrollee utilization of long-term services and supports.
