Job summary

Commonwealth Care Alliance is seeking an Integrated Care Clinical Manager in Massachusetts. The RN will provide monitoring, follow-up, and clinical care management to dually-eligible enrollees with complex medical, behavioral, and social needs.

The role emphasizes integrating health services and coordinating resources to improve outcomes and quality of life. The incumbent will lead the ICT, engage with enrollees in home and community settings, and collaborate with providers to ensure seamless

Qualifications

• Current, unrestricted RN license required.
• Associates degree in nursing from an accredited program.
• Experience in care coordination or dual-eligible populations preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage enrollees in home and community settings to manage complex care.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate with providers and community resources to ensure seamless care transitions.
• Assess and monitor enrollees using standard assessments and plans.
• Educate enrollees and families about benefits, chronic disease management and preventive care.
• Travel to member homes and sites as required to support care management.
• Promote adherence and coordinate with primary care and specialists.
