Qualifications

• Minimum of two years of nursing or case management experience required.

Responsibilities

• Conduct nursing assessments for PACE eligibility.
• Explain the PACE program to prospective participants.
• Coordinate home visits and education for participants.
• Maintain accurate participant information and documentation.
• Support quality improvement initiatives in the program.
