Qualifications

• Associate degree or equivalent in a healthcare/related field.
• Current RN license active in Nebraska.
• 2+ years in medical services or clinical setting with intake exposure.
• Home health experience preferred.

Responsibilities

• Acts as a liaison interfacing with referral sources and field staff.
• Receives and coordinates all incoming calls and basic service information.
• Communicates arrangements for initiation of care/services with patients and families.
• Completes intake screening and preliminary admission decision.
• Logs referrals and conducts data entry for customers seeking care.
• Promotes care/services to prospective patients/clients.
• Assists with verifiable orders and PHI handling as allowed by law.
• Supports clinical team to meet standards of nursing practice and regulations.
• Participates in community outreach to grow patient base.
• RN home health duties including occasional visits and on-call.
• Performs other assignments as requested.
