Job summary

Mercy STL Home Health is seeking an experienced Registered Nurse for the Intake role. The position supports Hospice, Home Health, and Home Infusion referrals in acute care, physician clinics, and select nursing homes or rehabilitation centers.

You will liaise with discharge planners, social workers, and physicians to ensure timely discharges and continuity of care within the Mercy Health System. Hours are 8am-4:30p, with multiple benefits.

Qualifications

• Minimum three years of nursing experience required.
• Experience in Home Health preferred.
• Inpatient acute and community settings experience preferred.

Responsibilities

• Accept Hospice, Home Health and Home Infusion referrals across applicable settings.
• Maintain an active presence with discharge planners, medical social workers, physicians, and future patients.
• Assist in facilitating timely discharges and support continuity of care within Mercy Health System.
