Qualifications

• RN license in VA or NC or Compact/Multi-state required.
• BLS certification within 90 days of hire.
• 1 year nursing experience required.
• 1 year acute care case management preferred.
• ED or ICU experience preferred.

Responsibilities

• Coordinate comprehensive care management for inpatients.
• Facilitate patient transitions to appropriate level of care.
• Collaborate with healthcare team and patient/caregiver for care management.
• Develop and implement care plans addressing physical, mental, and psychosocial needs.
