Sentara Leigh Hospital in Norfolk, VA is hiring an Inpatient Case Manager/Registered Nurse for the Emergency Department. The role is a full-time, 36-hour position with a day shift 9:00 AM–9:00 PM EST, Wednesday through Friday.

A sign-on bonus up to $20,000 is offered for qualified candidates. The RN will manage transitions, coordinate care, and support hospital patients requiring comprehensive case management.

Qualifications

• 1 year nursing experience required.
• 1 year acute care case management preferred.
• ED/ICU experience preferred.

Responsabilités

• Provide and facilitate comprehensive care management across the healthcare continuum with focus on patient transitions.
• Support hospitalized patients identified for case management needs and coordinate care with the health team.
• Promote efficient use of health services and monitor care transitions and LOS.
• Develop and implement a care plan addressing physical, mental and psychosocial needs.
