Sentara Princess Anne Hospital is hiring an Inpatient Case Manager/Registered Nurse (RN) for the Emergency Department (ED) on-site in Virginia Beach, VA. This full-time role follows a 4x10 schedule from 9:00 AM to 7:30 PM, Sunday through Wednesday or Wednesday through Saturday.

A sign-on bonus up to $20,000 is available for qualified candidates. You will lead comprehensive care management, support patient transitions, and collaborate with the care team to ensure appropriate level of care and

Qualifications

• RN license in VA, NC or compact/multi-state required.
• Bachelor's Degree in Nursing preferred.
• 1 year nursing experience required; acute care case management preferred.
• BLS certification required within 90 days of hire.
• Case management certification within one year of eligibility.

Responsabilités

• Lead comprehensive care management and patient transitions.
• Promote appropriate utilization and monitor LOS with the care team.
• Develop and implement care plans addressing physical, mental and psychosocial needs.

Connaissances

RN license (VA/NC/Compact)

1 year nursing experience

1 year acute care case management

ED/ICU experience

BLS certification

Case management certification (ACM/CCM
