Sentara Albemarle Medical Center in Elizabeth City, NC is hiring an Inpatient Case Manager RN for the Emergency Department. This full-time role focuses on care management, patient transitions, and ensuring clinical appropriateness and appropriate length of stay.

Requirements include BSN preferred, NC/compact RN license, BLS within 90 days, and ACM/CCM certification within one year. ED or ICU experience and 1 year of acute care case management are preferred.

Qualifications

• Bachelor's Degree in Nursing is preferred.
• Registered Nurse license in NC/VA/Compact required.
• BLS within 90 days of hire.

Responsibilities

• Facilitate comprehensive care management for inpatients.
• Develop and implement care plans addressing physical, mental, and psychosocial needs.
• Coordinate with healthcare team to manage transitions and LOS.
