Job summary

Mercy is seeking an RN Care Manager to coordinate discharge planning and ensure patients receive appropriate care. The role involves collaborating with physicians, nurses, and other professionals to support patient needs and promote safe transitions from admission to discharge.

Ideal candidates have a BSN and an active RN license, with 2–3 years in acute care and care/Utilization Management experience preferred. BLS certification is required at hire or within 90 days.
