Mercy is seeking a Care Manager to coordinate discharge planning, support safe transitions, and navigate patient journeys from admission through discharge. You will work with physicians, nurses, and other professionals to ensure timely, coordinated care across the patient population.

Preferred candidates have 2–3 years of acute care hospital experience, BSN, and a current RN license. BLS is required; Case Management certification is preferred.

Qualifications

• BSN required and current RN license.
• 2–3 years acute care hospital experience preferred; care/Utilization Management experience preferred.
• BLS required at hire or within 90 days; Case Management certification preferred.
