Job summary

Oak Street Health in Columbus, OH is seeking a Registered Nurse to coordinate patient care and deliver high-quality nursing services across the care team. You will assess patients, triage, mentor staff, and support care plans in a fast-paced clinic environment.

Qualifications

• Active RN licensure in good standing with the applicable state.
• BLS Certification.
• Electronic Medical Record (EMR) experience.
• US work authorization.

Responsibilities

• Provide competent nursing care following standardized protocols.
• Respond to telephonic requests and coordinate care.
• Provide triage and symptom management to patients.
• Review medications and update as needed.
• Provide education on chronic conditions and care plans.
• Collaborate with care team to monitor and progress plans.
• Build relationships with patients and caregivers to support preventive care.
• Create welcoming, engaging environments for patients and teams.
• Delegate tasks to care team members as needed.
• Participate in care team meetings to discuss patient care and operations.
• Deliver an exceptional patient experience through service and respectful care.
• Perform point-of-care testing and phlebotomy as needed.
• Perform other duties as assigned.
