Oak Street Health in Chicago is seeking an experienced Registered Nurse to coordinate patient care and deliver timely, clinically competent nursing services. You will work with a multidisciplinary team to implement care plans, perform triage, and educate patients about chronic conditions.

This full-time role requires active RN licensure, BLS, and EMR experience, with US work authorization. Strong communication and patient-centered care are essential, with Spanish-speaking ability a plus.

Qualifications

• Active RN licensure in good standing.
• BLS certification required.
• Experience with Electronic Medical Records.
• Ability to maintain patient confidentiality.
• US work authorization.
• Ability to assess patients without face-to-face interaction; strong communication.

Responsabilités

• Provide competent nursing care safely and consistently.
• Respond to telephonic requests in a dependable manner.
• Triage and manage symptoms for patients in various settings.
• Review medications and update as needed.
• Deliver education and direct patient care for chronic conditions.
• Collaborate with the care team to implement care plans.
• Create a welcoming environment and delegate tasks as needed.
• Participate in care team meetings and clinic operations.
• Deliver an exceptional patient experience with respectful care.
• Perform point-of-care testing and phlebotomy as needed.
• Other related duties as assigned.
