Job summary

Oak Street Health is seeking a Registered Nurse to support the care team within a value-based care model. The Center Nurse leads nursing assessments, chronic condition management, and education for patients and families, reporting to the Practice Manager.

Responsibilities include triage, wound care, vaccines, medication management, and onsite testing. Strong collaboration with providers and a patient‑centered approach are essential.

Qualifications

• Active, unrestricted RN licensure in the state of residence.
• BLS or CPR certification.
• Fluent written and spoken English.
• US work authorization.
• EMR experience.
• 2+ years of healthcare experience as an RN.

Responsibilities

• Support patient assessment and triage to ensure appropriate care and avoid unnecessary hospital visits.
• Execute protocol-driven chronic condition management.
• Perform wound care and medication administration (including vaccines).
• Educate patients and caregivers about chronic conditions and preventative care.
• Review medications during nurse-led visits for safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs, specimen collection).
• Coordinate with providers to manage complex care plans and weekly transitions from hospital to clinic.
• Delegate tasks to medical assistants and phlebotomists to support flow.
