Job summary

Oak Street Health is seeking a Registered Nurse to coordinate care and provide compassionate nursing services across our patient care teams. You will assess, educate, and support patients with a focus on high-quality outcomes and patient experience, using standardized care pathways and EMR for documentation.

The role emphasizes teamwork, careful triage, and coordination with providers to ensure timely, effective care and a welcoming clinic environment in a full-time setting.

Qualifications

• Active Registered Nurse licensure in good standing with the applicable state.
• BLS certification.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and handle information confidentially.
• US work authorization.
• Ability to assess patients without face-to-face interaction and communicate effectively.

Responsibilities

• Provide competent nursing care following standard protocols.
• Respond to telephonic requests and ensure timely, responsive service.
• Provide triage and symptom management for patients remote or in person.
• Manage medications, prescriptions, refills, and prior authorizations using EMR.
• Review patient medications and update as needed.
• Educate patients and care partners, including chronic condition management.
• Collaborate with the care team to execute and monitor care plans.
• Build relationships with patients and caregivers to support preventative care and ED/hospital diversion.
• Create a welcoming environment through effective team communication.
