Oak Street Health is seeking a Registered Nurse (Bilingual Spanish Required) for a Bronx, NY center. You will coordinate patient care, provide nursing assessments and education, and collaborate with a multi-disciplinary care team to deliver exceptional patient experiences.

Strong RN licensure, BLS, EMR familiarity, and US work authorization are required. Spanish language skills and prior clinic experience are highly valued in a fast-paced, patient-centered environment.

Formación

• Active RN licensure in good standing.
• Current BLS certification.
• Experience using electronic medical records.
• Ability to maintain patient confidentiality and privacy.
• Authorized to work in the US.

Responsabilidades

• Provide competent nursing care in a safe, consistent manner.
• Respond to telephonic requests and triage patient needs.
• Perform triage and symptom management, including non-face-to-face care.
• Review and manage medications and prior authorizations as needed.
• Educate patients and care partners on chronic conditions and self-care.
