Job summary

Oak Street Health seeks a Registered Nurse (Bilingual Spanish Required) in Bronx, NY to coordinate patient care and deliver high-quality nursing services in a center setting.

As an RN, you will use standardized care paths, conduct assessments, and educate patients while collaborating with the care team to improve outcomes and patient experience. Fluency in Spanish is preferred and the role reports to the Practice Manager or Nurse Supervisor.

Qualifications

• Active RN licensure in good standing
• BLS certification required
• Experience with Electronic Medical Records (EMR) systems
• Ability to protect patient confidentiality
• Authorization to work in the US
• Strong communication and assessment skills, including remote interactions

Responsibilities

• Provide competent nursing care and deliver safe, consistent services
• Respond to telephonic requests and ensure timely responsiveness
• Perform triage and symptom management via telecommunication or on-site
• Follow standardized protocols for medications, refills, and prior authorizations
• Review and update patient medications and care plans
• Educate patients and deliver direct care in person, by phone, or in groups
• Collaborate with the care team to monitor and progress care plans
• Build relationships with patients and caregivers to support preventive care
• Create a welcoming environment for patients, families and teams
• Delegate tasks to the care team to meet patient needs
• Participate in care team meetings to discuss patient care and clinic operations
• Deliver an exceptional patient experience through service and respect
• Perform point-of-care testing and phlebotomy as needed
• Perform other related duties as assigned
