Oak Street Health is seeking a Registered Nurse to build strong patient relationships by coordinating care and delivering a seamless experience for patients and their support teams. You will apply standards, guidelines and pathways to ensure timely, competent nursing care and engage the care team through clear communication and delegation.

You’ll report to the Practice Manager or Nurse Supervisor, participate in care team meetings, and strive to create a welcoming environment.

Formación

• Active RN licensure in good standing in the applicable state.
• BLS Certification is required.
• EMR experience and ability to maintain patient confidentiality.

Responsabilidades

• Provide competent nursing care following standards and pathways.
• Respond to telephonic requests and be responsive to patient needs.
• Provide triage and symptom management, with or without in-person contact.
• Review medications and manage prescriptions and prior authorizations.
• Educate patients and care teams to improve outcomes.
• Collaborate with the care team and monitor care plan progress.
• Deliver an exceptional patient experience through service and respectful care.

Conocimientos

RN licensure in good standing
