Job summary

Oak Street Health in Lancaster, PA seeks a dedicated Registered Nurse to coordinate patient care and deliver high-quality nursing in a fast-paced clinic. The RN will support chronic disease management and collaborate with the care team to ensure timely, safe, and compassionate care.

The role requires PA RN licensure, CPR/BLS, EMR experience, and a strong emphasis on patient confidentiality, communication, and teamwork. 40 hours weekly with a comprehensive benefits package is provided.

Qualifications

• Active Registered Nurse licensure in good standing in Pennsylvania.
• Active CPR and/or BLS Certification.
• Experience with Electronic Medical Records (EMR).
• Clinic setting experience with hands-on patient care.
• Ability to maintain patient confidentiality under HIPAA.

Responsibilities

• Provide competent nursing care in a safe, consistent manner.
• Respond to telephonic requests and support patient needs.
• Provide triage and symptom management for patients on/off site.
• Utilize protocols for medications, refills, and auths.
• Review patient medications and update as needed.
• Educate patients about chronic conditions and care plans.
