Job summary

YAI is seeking a Registered Nurse to provide compassionate, person-centered nursing care to individuals with I/DD in New York programs. The RN collaborates with direct support and leadership to assess needs, create and monitor care plans, and ensure compliant nursing care.

Responsibilities include triaging calls, updating plans of nursing care, documenting medical information, and training staff on medical needs and medications. Strong clinical judgment and communication are essential.

Qualifications

• Valid, active RN license and registration.
• Current Basic Life Support (BLS) certification for Healthcare Providers.
• Demonstrates sound clinical judgment in resolving medical concerns.
• Excellent interpersonal, verbal and written communication skills.
• Ability to lead, direct and teach program staff on medical support for persons supported.
• Proficiency with electronic systems including EHR and Workday; MS Office.

Responsibilities

• Provide nursing services for persons supported in assigned programs in collaboration with staff and leadership.
• Develops and maintains understanding of regulations related to medical and nursing care.
• Uses the nursing process to assess, set goals, intervene and evaluate care.
• Educates on health care, treatments, medications, and healthy lifestyles.
• Document calls, encounters and medical information in required systems.
