Job summary

YAI is seeking a Registered Nurse to provide compassionate, person-centered nursing care to individuals with I/DD and others supported in assigned programs. Under supervision, the RN assesses needs, creates and monitors nursing care plans, and collaborates with staff and leadership to ensure high-quality medical support.

The role emphasizes patient education, coordination of care, medication administration, and maintaining accurate documentation within EHR and eMAR systems.

Qualifications

• Valid, active RN license in assigned state(s) of operation (NY or NJ) and BLS certification.
• Excellent interpersonal, verbal and written communication skills.
• Proficiency with electronic health records and Workday.

Responsibilities

• Provide nursing services per plans of care and coordinate with staff and leadership.
• Develop and maintain plans of nursing care and oversee medication administration.
• Assess patient conditions, provide education on treatments and healthy lifestyles.
• Triage calls and determine appropriate actions for urgent needs.
• Maintain documentation in EHR/eMAR and ensure timely reporting.
