Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Registered Nurse to provide rapid response hospice field visits across Palm Beach County. Qualifications

• Active Florida RN license and BLS certification.
• One year of public health, home nursing, oncology, medical/surgical, geriatric, or hospice nursing preferred.
• Ability to triage and manage urgent health conditions in non-clinical settings.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide rapid response hospice field visits throughout Palm Beach County. If you are driven to deliver meaningful, patient-centered care during critical moments, this role offers the opportunity to truly change lives—one visit at a time. You will work the following schedule: every Fri/Sat/Sun from 7am - 7:30pm.

What You'll Do

• Be responsible for assessing, triaging, and managing urgent and emergent health conditions outside of traditional clinical environments.
• You will operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.
• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed.
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
