Job summary

Trustbridge Hospice, a part of Empath Health, is seeking a Per Diem Lead RN to ensure high-quality patient care by managing EMR workflows and providing clinical leadership. The role supports patients, providers, and interdisciplinary teams from admission through discharge.

Qualifications

• RN degree from ACEN/CCNE-accredited program.
• Florida RN license active and in good standing.
• BSN preferred but not required.
• Current BLS certification required.
• Hospice certification preferred.
• 1 year hospice/oncology/medical-surgical/home health/public health nursing preferred.

Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated Per Diem Lead RN to help ensure the delivery of high-quality, coordinated patient care through comprehensive management of Electronic Medical Record (EMR) workflows and effective clinical leadership. This role supports patients, providers, and interdisciplinary teams by promoting accuracy, collaboration, and continuity of care from admission through discharge.

This position will be located in our Boca Raton office working the following schedule: Every Saturday & Sunday 5:00am - 1:30pm

• Electronic Medical Record Management: Process and manage all Electronic Medical Record (EMR) workflows from admission through discharge, ensuring accurate, timely, and complete documentation that supports continuity and quality of care.
• Clinical Leadership: Serve as a professional clinical leader in assessing, planning, guiding, and evaluating patient care, applying sound nursing judgment and evidence-based practice to support positive patient outcomes.
• Interdisciplinary Collaboration: Interact effectively with management, physicians, and field staff using a teamwork-focused approach to coordinate care, resolve issues, and ensure alignment with organizational goals and patient care standards.

What you'll Need

• Registered Nurse (RN) degree from a college or university accredited by the Accreditation Commission for Education in Nursing (ACEN) and/or the Commission on Collegiate Nursing Education (CCNE) preferred.
• Currently licensed and registered as a Registered Nurse in the State of Florida.
• Bachelor of Science in Nursing (BSN) preferred.
• Current Basic Life Support (BLS) Certification required.
• Hospice Certification preferred.
• One year of recent experience in hospice, oncology, medical/surgical, home health, or public health nursing preferred.
• Demonstrated expertise in support skills, palliative care principles, and patient/family education techniques.
• Flexible, creative, assertive, articulate, nurturing, and self-nurturing professional demeanor.
• Experience with Homecare Homebase software a plus.
• Strong computer skills required, including proficiency with clinical documentation systems.
• Ability to work effectively under stress and meet deadlines in a fast-paced environment.
