Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Registered Nurse to provide rapid response overnight in-home care across Broward County. The role supports hospice patients and families, working Fri/Sat/Sun 7am–7:30pm in independent practice with a focus on comfort, dignity, and responsive care.

The RN will collaborate with the attending and hospice physicians, manage care plans, and advocate for patient well-being in urgent situations, contributing to Full Life Care.

Qualifications

• RN degree from an ACEN/CCNE-accredited program.
• Current Florida RN license.
• BLS certification.
• 1 year in public health, home nursing, oncology, medical/surgical, geriatric, or hospice nursing preferred.
• Reliable transportation with valid Florida driver’s license.
• Basic computer skills; EMR experience a plus.

Responsibilities

• Provide rapid response overnight field visits for hospice patients in Broward County.
• Coordinate patient care with attending physicians and the IDG.
• Perform comprehensive assessments identifying physical, emotional, and psychosocial needs.
• Develop and manage the Interdisciplinary Group (IDG) Care Plan.
• Ensure timely implementation of interventions and services in the care plan.
• Deliver compassionate, in-home care and advocate for patient comfort and dignity.
