Job summary

Trustbridge Hospice, a member of Empath Health, seeks an RN Case Manager to coordinate care for patients and families across Southern Palm Beach County. The role supports comfort and quality of life with the interdisciplinary team, working 8:30am–5pm and every other weekend.

Licensed Florida RN with BLS is required; hospice care and care coordination experience are preferred. Strong communication and teaching skills are essential for success in this mission-driven team.

Qualifications

• Florida-licensed RN with 1 year in hospice, home health, oncology, or geriatric nursing.
• Current Basic Life Support (BLS) certification.
• Experience in hospice nursing and care coordination preferred.
• Strong communication and teaching skills; ability to guide patients and families.

Responsibilities

• Coordinate care for patients and families across the hospice journey.
• Advocate for patient needs and coordinate with the interdisciplinary team.
• Develop and implement individualized plans of care with the IDG.
• Monitor patient conditions, deliver interventions, and provide education.
• Address concerns promptly to ensure satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated RN Hospice Case Manager to join our team serving patients and families across Southern Palm Beach County. In this role, you’ll have the unique opportunity to provide comfort, dignity, and support to individuals facing advanced illness—while working alongside a collaborative, mission-driven team. Your schedule would be: 8:30am - 5pm & every other weekend.

What You’ll Do

As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. You will:

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
