Job summary

Empath Health's Trustbridge Hospice in Palm Beach County is seeking a caring Hospice Registered Nurse (RN) Case Manager. You will coordinate patient-centered care with the interdisciplinary team, develop individualized plans, and guide families through the journey of advanced illness.

This role requires a Florida RN license, BLS certification, and experience in hospice or related fields. Expect a Monday–Friday schedule with a weekend requirement and a strong commitment to Full Life Care for All.

Qualifications

• Florida RN license required.
• BLS certification required.
• Experience in hospice, public health, home health, oncology, medical/surgical, geriatric, or hospice nursing preferred.
• Knowledge of hospice case management, crisis intervention, communication, teaching, and thanatology concepts.

Responsibilities

• Advocate for patients and families; ensure needs are addressed.
• Coordinate care under the direction of the primary physician with the interdisciplinary team.
• Develop and implement individualized care plans as part of the IDG.
• Monitor patient conditions, provide interventions, and deliver education.
• Address concerns promptly to ensure satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.
Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

What You'll Do

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
