Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate RN to join the Gerstenberg Inpatient Care Center team in West Palm Beach. The role involves coordinating care with the physician and IDG, performing patient assessments, and ensuring timely delivery of interventions to support comfort and quality of life.

You will monitor patient conditions, provide education, and address concerns to uphold dignity and trust.

Qualifications

• Florida RN license must be current.
• BLS certification required.
• Minimum 1 year in public health, home health, oncology, medical/surgical, geriatric, or hospice nursing preferred.
• Understanding of hospice nursing: case management, crisis intervention, teaching, thanatology.
• Reliable transportation and a valid Florida driver’s license.

Responsibilities

• Coordinate patient and family care with the primary physician and IDG.
• Conduct thorough assessments of patient and family needs.
• Ensure timely delivery of interventions and services per the care plan.
• Monitor patient conditions and provide education to enhance comfort and quality of life.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to join our Gerstenberg Inpatient Care Center team in West Palm Beach. • Coordinate patient and family care in collaboration with the primary physician and interdisciplinary team (IDG)
• Conduct thorough assessments of patient and family needs
• Ensure timely and coordinated delivery of interventions and services outlined in the care plan
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs.
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
