Job summary

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate Registered Nurse (RN) to provide rapid-response hospice field visits throughout Palm Beach County. The role follows a 6:00am - 4:30pm schedule with every other weekend, offering meaningful, patient-centered care during critical moments.

The ideal candidate is a Florida-licensed RN with BLS, and has experience in public health, home nursing, hospice, or related fields.

Qualifications

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills and needs.
• Basic computer skills and EMR software experience a plus.

Responsibilities

• Provide rapid-response, patient-centered clinical care in home and community settings.
• Assess, triage and manage urgent and emergent health conditions with autonomy.
• Collaborate with physicians and interdisciplinary teams to prevent ED visits or hospitalizations.
• Advocate for patients and families, addressing needs.
• Coordinate comprehensive care under the direction of the primary physician.

What You’ll Do

The Field-Based Emergent Care Registered Nurse (RN) provides rapid-response, patient-centered clinical care to individuals in home and community settings. You will:

• Be responsible for assessing, triaging, and managing urgent and emergent health conditions outside of traditional clinical environments.
• You will operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.
• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.

• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
