Job summary

Chapters Health is seeking a dedicated RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and deliver clinical, palliative, and supportive care to keep patients in their homes where possible.

The role requires an active RN license in Florida, at least one year in hospice or hospital settings, and preferred Alzheimer’s/Dementias training. Proficiency with EMR/EHR and a valid driver’s license is essential.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one year nursing experience in hospice or hospital preferred.
• Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs (preferred).
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance per company policy.

Responsibilities

• Provide direct care to patients and families as part of the Interdisciplinary Team (IDT) with appropriate nursing interventions.
• Coordinate the Plan of Care reflecting patient’s evolving needs.
• Educate patients and families about disease process, palliative care, safety, and care giving.
• Visit participants in their homes; assess home safety and medication/nutritional compliance.
• Report changes to IDT, participate in IDT meetings, and communicate with physicians and staff.
• Initiate skilled nursing interventions to prevent complications and maximize comfort.
• Perform other duties as assigned.
