Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to help patients remain in their homes whenever possible.

The role requires an active RN license and at least 1 year of nursing experience; hospice or hospital experience is preferred. EMR/EHR experience and strong communication are essential.

Qualifications

• Current RN license in the state where the employee will be working.
• Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
• Experience with an EMR/EHR system.

Responsibilities

• Assess and identify patient/family needs and coordinate the Plan of Care with the Interdisciplinary Team (IDT).
• Provide clinical, palliative and supportive care to patients to maximize home care.
• Coordinate in‑home visits and communicate with IDT and physicians.
