Job summary

Chapters Health System is seeking a Registered Nurse (Case Manager) to assess patient/family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and deliver clinical, palliative, and supportive care to keep patients at home when possible.

The role requires an active RN license, hospice or hospital experience, and ability to visit patients at home, with a focus on compassionate, patient-centered care within the Chapters Health network.

Qualifications

• Current RN license in the state of employment.
• Hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance.
• Ability to visit participants in their homes.

Responsibilities

• Assess patient and family needs and coordinate Plan of Care with IDT.
• Provide clinical, palliative, and supportive care.
• Communicate with physicians and IDT; SBAR documentation.
