Job summary

Optum is seeking a Registered Nurse Case Manager to take full nursing responsibility for hospice patient care under the Plan of Care. You will deliver high-quality, compassionate care and adapt to each home setting while aligning with hospice policies and objectives.

Responsibilities include initial eligibility evaluation, ongoing assessment every 15 days, plan-of-care initiation, and close collaboration with patients, families, and the care team to ensure optimal outcomes.

Qualifications

• Current, unrestricted RN licensure in the state of practice.
• Active driver's license, valid auto insurance, and access to reliable transportation.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Assess patient and family needs and update plans of care with appropriate documentation.
• Develop and coordinate the plan of care with the IDG while maintaining accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician, and other IDG members daily.
