Job summary

Optum is seeking a Per Diem Registered Nurse Case Manager to deliver hospice care under the Plan of Care, ensuring timely evaluations and high-quality patient outcomes. You will document care plans, coordinate with the IDG, and maintain accurate clinical notes during visits.

Responsibilities include initial eligibility assessments within 48 hours of referral, ongoing re-assessments every 15 days, and collaboration with families, physicians, and the care team to meet patient needs.

Qualifications

• Current unrestricted RN licensure in the state of practice (MS).
• 1+ years of clinical experience.
• Current driver's license, valid insurance, and access to reliable transportation.
• MS state requirement: 1 year RN experience, LPN substitution allowed per policy.
• CPR certification preferred (within 90 days of hire).

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral
• Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days
• Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care
• Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily
