Optum is seeking a Registered Nurse Case Manager for Elite Hospice to deliver the Plan of Care for hospice patients. You will evaluate performance, ensure quality care, and support hospice goals within the patients' homes and communities.

The role involves initial nursing assessments within 48 hours of referral, ongoing reassessments, plan of care initiation, and collaboration with families, physicians, and the IDG to ensure comprehensive patient care.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to complete within 90 days of hire.
• 1+ years of clinical experience.

Responsabilités

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family response.
• Collaborates with patient/family, attending physician, and IDG members for daily care.
• Function in diverse home situations regardless of age, race, creed, color, sex, disability, or financial condition.
