Job summary

Elite Hospice, part of LHC Group and a member of the Optum family, is seeking a Registered Nurse Case Manager to lead the plan of care for hospice patients. You will evaluate performance and collaborate with professionals to implement treatment strategies that support physical, psychosocial, emotional, and spiritual well-being.

Responsibilities include initial nursing evaluations within 48 hours of referral, ongoing reassessments every 15 days, and coordination of the care plan.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• 1+ years of clinical hospice or related care experience preferred.

Responsibilities

• Makes initial nursing evaluation in determining eligibility for hospice within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs; reassesses at least every 15 days.
• Initiates and coordinates the plan of care; documents goals, interventions, and responses.
