Job summary

Ultipro is seeking a Home Care Supervisor RN to coordinate and supervise home care services for PACE participants in Modesto and Merced counties. You will manage staff, ensure regulatory compliance, and oversee care planning from center to home environments.

The role requires an active CA RN license, two years of supervisory health care experience, and a BSN or higher. Competitive hourly compensation, benefits, and a focus on quality of care are provided.

Qualifications

• Current BLS CPR Card
• Minimum two years supervisory or management experience in a health care setting
• Minimum two years’ experience working with the frail or elderly population
• Current unencumbered CA Registered Nurse (RN) License
• Bachelor’s degree or higher in nursing or other health related field

Responsibilities

• Manage home care employees and scheduling; arrange coverage for call outs
• Reconcile vendor invoices and approve payments
• Coordinate 24-hour home care services to meet Plan of Care goals
• Supervise staff and contracted services; evaluate performance and quality of care
• Provide after-hours support to clients and referral partners
• Develop policies and procedures for Home Care Services
• Onboard and coach new home care staff; oversee regulatory compliance
• Oversee assessments, care planning, and regulatory requirements in home care
• Monitor service delivery and implement corrective actions to address gaps
• Support regulatory audits and EMR upgrades impacting home care workflows
• Educate staff on EMR workflows and documentation standards
• Manage staff performance, payroll reconciliation, and scheduling
