Description

Under the direct supervision of the PACE Director of Center Operations RN and the Home Care Administrative Manager, the Home Care Clinical RN is responsible for the development and implementation of homecare nursing home/boarding care services for program participants and oversight of all clinical skilled home care services. Supervision of Personal Care Assistants (PCA) and Licensed Vocational Nurses (LVN) delivery assistance with participant’s activities in the center and home environments as assigned.

Until our Stockton location is operational, the role will be based and trained at our Modesto center.

Schedule is Monday - Friday, 8:00am - 5:00pm

Duties and Responsibilities

  • Assesses, using the nursing process, the home care needs of a frail elderly population, and identifies and develops specific plans of care. Initial and periodic assessments to be conducted minimally every six (6) months and must be completed prior to the scheduled team meeting.
  • Responsible for the provision of home care to meet identified participant outcomes, and to achieve the goals as outlined in the participants Plan of Care
  • Communicates participant changes with team members, coordinates and authorizes all Durable Medical Equipment.
  • Coordinates and supervises care plans for participant that are residing in the nursing home/boarding home. Includes interaction with other facility staff.
  • Supervise the delivery of skilled services and personal care services in the participant’s home of our staff and quality of contracted services in the home.
  • Coordination of 24 hour care delivery and the implementation of all home care services which include nursing, PT, OT, ST and personal care services to ensure that quality services are provided to meet participant needs.
  • Performs evaluations of Home care team members, and resource management of home care team staff in conjunction with Center Manager.
  • Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.
  • Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
  • Ensures all regulations and standards are up to date, keeping compliance with orders or directives issued by constituted government/regulatory agencies. Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Supervise PCAs and LVNs in accordance with regulations.
  • Other projects and duties as assigned.

Physical Demands

  • Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
  • Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
  • Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
  • Must have high manual dexterity.
  • Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
  • Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
  • Subject to unpleasant odors.
  • The noise level is usually quiet to moderate, but may at times be noisy and crowded.

Minimum Qualifications

  • Valid CA Driver’s License, acceptable driving record, and vehicle insurance.
  • Must be able to work effectively in a team environment. Must be able to treat geriatric patients.
  • Only act within the scope of his or her authority to practice.
  • Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, perform nursing duties, be flexible, innovative, focus on results and travel regularly.
  • Meet a standardized set of competencies established by PACE and approved by CMS before working independently.

Education/Experience:

  • High school diploma or equivalent. Graduate of an accredited school of professional registered nursing.
  • Current unencumbered CA Registered Nurse (RN) License.
  • Current BLS CPR Card certified by American Heart Association.
  • A registered nurse with two (2) years nursing experience within the last five (5) years in a home health agency, primary care clinic or health facility.
  • Minimum one (1) year working with the frail or elderly population.
