Job summary

Redwood Coast PACE, operated with Humboldt Senior Resource Center, delivers comprehensive medical, social, nutritional, rehabilitation, and support services to eligible older adults and aims to help them remain healthy and independent at home.

The Home Care Manager in Fortuna coordinates in-home services for PACE participants, oversees a multidisciplinary CarePartner team, and works with agencies to ensure safe, high-quality care for frail elders.

Qualifications

• BSN preferred
• California RN or LVN license required
• 2+ years RN/LVN experience required
• 1+ year supervisory experience required
• Driver’s license and personal vehicle required
• CPR and First Aid certification within 6 months of hire

Responsibilities

• Oversee day-to-day home care services for PACE participants in the Fortuna area.
• Supervise Home Care Assistants, Home Care Schedulers, Home Care Supervisors, and Lead CarePartners.
• Coordinate hiring, orientation, training, supervision, and performance management of team members.
• Oversee contracted home care agencies and in-home CarePartners.
• Assess home care needs of frail elderly participants.
• Conduct home safety visits as assigned.
• Coordinate durable medical equipment and related services.
• Ensure assessments are completed and documented accurately and on time.
• Support quality assurance and performance improvement initiatives.
• Address participant rights violations, grievances, appeals, and quality-of-care concerns.
• Participate in the Redwood Coast PACE Management team.
• Provide on-call coverage in coordination with the Home Care team.
• Collaborate with staff, contractors, and community partners to support PACE goals

Requirements to Apply

• Current California RN or LVN license
• At least 2 years of RN/LVN experience
• At least 1 year of experience working with medically frail or elderly populations
• At least 1 year of supervisory experience
• Personal vehicle with required insurance
• CPR and First Aid certification within 6 months of hire
• Bachelor of Science in Nursing preferred

Job Overview

The Home Care Manager oversees and coordinates home care services for PACE participants in the Fortuna area. This role supervises the in-home CarePartner team, coordinates home safety assessments and durable medical equipment, and works closely with home care agencies and interdisciplinary teams to ensure high-quality, safe, and participant-centered care.
