United Health Centers of the San Joaquin Valley seeks a PACE Registered Nurse (RN) to join our new PACE program in Fresno, CA. You will assess, coordinate, and deliver clinical care to our elderly participants under primary care supervision.

This role emphasizes team collaboration with the IDT, medication management, wound care, and patient education. We offer competitive hourly wages starting at $42.87 and a comprehensive benefits package.

Qualifications

• CA RN license required and ACLS certification
• At least 1 year of RN experience; elderly care preferred
• Bilingual English/Spanish preferred

Responsabilités

• Assess, coordinate, monitor, and provide nursing care for PACE members.
• Triage and respond to emergencies; participate in care planning with IDT.
• Administer medications and manage medication systems per clinical standards.
• Document nursing care in medical records promptly and accurately.
• Mentor new staff and assist in orientation.
• Schedule appointments and coordinate visits with MDs and NP.
