Central Valley PACE is seeking a Home Care Clinical RN in Stockton to develop and supervise homecare nursing services for program participants. You will oversee PCAs and LVNs, coordinate care with the interdisciplinary team, and ensure adherence to CMS and regulatory standards.

The role involves lead responsibility for care planning, regular assessments, and collaboration with the Center Manager to optimize participant outcomes in home and nursing facility settings.

Qualifications

• Patients require geriatric care, with nursing processes and care planning.

Responsibilities

• Assess home care needs of frail elderly population and develop plans of care.
• Oversee delivery of skilled nursing and personal care services.
• Coordinate 24-hour care delivery and manage team assignments.
• Supervise PCAs and LVNs ensuring regulatory compliance.
• Communicate changes in participant condition to the care team.
