Job summary

Central Valley PACE is seeking an RN Supervisor for the Merced, Modesto, and Stockton PACE Centers to provide clinical and operational oversight of the LOC enrollment process and to conduct assessments and audits for participant eligibility.

The role leads LOC nursing staff, ensures DHCS LOC compliance, coordinates daily operations, and supports quality improvement with the IDT to deliver high-quality, patient-centered care.

Qualifications

• Valid CA RN license and BLS CPR.
• Minimum three years RN experience, home or community health preferred.
• Two years with frail or elderly population.
• One year supervisory experience.
• Ability to train, lead and supervise effectively.
• Strong communication and record-keeping skills.
• Must complete DHCS LOC training within 3 months.

Responsibilities

• Provides clinical and operational oversight of the LOC enrollment process.
• Supervises LOC nursing staff and provides coaching and performance feedback.
• Coordinates LOC meetings with the IDT team.
• Ensures regulatory compliance with California DHCS LOC requirements.
• Leads quality improvement initiatives and audits for participant care.
• Maintains accurate records and participates in staff development.
